Bereavement cards for hospice and care teams

Cards for a year of follow-up

Medicare-certified hospices make bereavement services available to the family for up to one year after the death of the patient (42 CFR 418.64(d)(1)). Many teams mark that year with a few short, well-timed notes. This group was put together for that.

• Soon after. “With sympathy.” Inside: “You are lifted in prayer and surrounded by love.”
• In the months between. Note cards with a few words: “May your memories bring you comfort ... and touch your world with peace.”
• On their birthday. “The beautiful thing about a memory is that it stays forever young in our hearts.” Inside: “Thinking of you on this anniversary of your loved one’s birthday and wishing you the gift of special memories.”
• On a wedding anniversary. Inside: “Caring thoughts surround you on your wedding anniversary.”

A few cards in this group mention prayer. Most do not. The wording is on every product page.

Two formats

• 5 x 7 cards with envelopes, in packs of six.
• 3.5 x 5 note cards, ten cards and ten envelopes to a package. These suit a short handwritten note.
